Background
Profile is a problem oriented EMR and provides five categories to store patient information:
- Diagnosis (Active & Inactive), Adverse, Procedure, Social History/Risk, and Admin.

1. Open patient’s Medical Record and then click on Problems.
Problem List – to enter or view in New Encounter
1. Open New Encounter and the Problems window appears in the left lower screen.
2. Right click in the Problems window to enter new Problems.
5 types of problems in the EMR
All past medical history can be stored a type of problem.

Diagnosis: Current Ongoing (chronic) problem should be entered as an ICD9 code
- To inactivate a Diagnosis Problem - change the status from active to inactive (see related help file below)
- Adverse: Used to enter allergies (or no Known Allergies) via the actions panel
- Procedures: Includes both operations and significant investigations
- Social/Risk: Used for social history and social risks. All family history is entered as a Risk type under SHX81. Enter a Staff Safety plan using SHX200.
- Adminstrative: Used by admin desk staff
If using a Problem Code as a Contact Title and wish to add the same Problem Code to the Problems List in a single click, check the "Add to Problems List" checkbox in the Contact Properties window.
