Reminder that MRPs should NOT be deleting diagnosis problems entered by others unless the diagnosis is incorrect.
A diagnosis that is no longer active should be marked as inactive. (see below) Inactive problems are by definition past history and should not be deleted.
While the MRP is most responsible for ensuring the accuracy of the medical record it is important to remember that other clinics/teams also rely on the information.
Some clinicians are “cleaning up” the problem list of some patients and deleting (instead of inactivating) a diagnosis such as syphilis. Clearly a diagnosis such as this should remain in the problem list and can be marked as an “Inactive” problem.
