Mark an Encounter Note as Entered in Error
Encounter notes should be marked as 'Entered in Error' when:
- Documentation entered into wrong client record
- Documentation entered under the wrong POS
Mark an Encounter Note/Contact as 'Entered in Error':
1. Go to Medical Record->Encounters
2. Locate and open the encounter that was entered in error
3. Click on the Contact title of the note to open the Contact Properties window
4. In Text Description field, type Entered in Error
5. Click OK
6. Remove any clinical documentation from the body of the encounter/contact and in its place document the reason the encounter is being marked as 'Entered in Error':
- Documentation entered on wrong client
- Documentation entered under wrong POS
7. Review the actions panel of the encounter to determine if there are any forms/referrals/prescriptions/etc. which need to be deleted as well. To delete items which were added to the encounter incorrectly follow the below steps:
7a. In the right-hand side of the encounter, right click on the item which needs to be deleted.
7b. Select Delete from the dropdown.
8. After marking the encounter as "Entered in Error' and removing all clinical documentation/forms/interventions/etc., save the encounter note.
Important: The encounter note that was entered in error is not deleted from the system. Instead, versions of that encounter note are saved in the system for auditing purposes. The most recent version (blank) displays when Reviewing Encounters.
View Encounter Notes Entered in Error
1. To review historical versions of an encounter note, click on the Version hyperlink
2. The Compare Transaction Versions window opens.
3. Drag and drop a version from the table on the left to the panels on the right to compare.
4. Highlighted text indicates differences between the versions.
