Background
New! Starting 13 Sep 2022, Profile EMR users can create new and modify existing rows in the Community Risk Screen tool within Profile EMR. Risk Screen data will continue to be stored in PARIS.
Use of the Community Risk Screen Tool (CRS Tool) and Control Plan is without exception, unless there is formal, program director approval to use another means of risk screening.
VCH recognizes that identification of hazards and assessment of risks, communication of those
risks and risks management is legislated under the WorkSafeBC Occupational Health & Safety
Regulation (OHS Regulation).
Risk Screening will ensure effective and consistent assessment of all risks related to community
clients receiving our services and/or work sites in which services are provided. It will promote
improved documentation and communication of risk among clinicians in all community programs. It will
assist in the mitigation of potential risk for the health and safety of staff.
Profile EMR CRS Tool
The CRS tool and control plan is a client-based document that is shared and accessible through Profile EMR and PARIS. Any changes to the CRS Tool in either Profile EMR and PARIS are viewable in either system. It is accessible to all VCH Community clinicians, who have access to Profile EMR or PARIS.
Documentation on the tool occurs during:
- Pre-Visit (prior to visit)
- Initial Visit
- Review Visits (ongoing basis)
A Risk Control Plan should be developed when a risk is identified. The identification of Risk(s) and a Control Plan will activate a “STAFF ALERT : SEE RISK SCREEN” alert, which will serve as notification to all staff, that they must reference the CRS Tool for more details.
Click the Reload Risk Screen from PARIS button to cancel a new or modified Risk Screen as this pulls the most recent Risk Screen information from PARIS. Once a new or modified Risk Screen is saved in Profile EMR, this information is sand saved into PARIS.

Note: Copying Risk Screen between family members is not a functionality supported in Profile EMR.
Launch the Risk Screen
To launch the Risk Screen, click on the Risk Screen icon on the Main Tool bar.
The Risk Screen consists of the following sections:
- Alerts grid
- Risk Screen grid
- Risk Screen summary
This help file is easily-accessed by clicking on the icon.
View Alerts
The alerts grid within the Risk Screen is a read-only view of the client's current alerts. The STAFF ALERT : SEE RISK SCREEN alert is auto-generated when Risk is identified and a Control Plan has been filled out.

View Risk Screen History
The Risk Screen grid shows all risk screens documented for the client. The most recently-recorded Risk Screen is loaded by default and documented information is viewable in the Risk Screen summary section just underneath the grid.
To view another Risk Screen, single-click on the relevant Grid row to view in the Risk Screen summary section.
Create a New Risk Screen
1. Click Create a New Risk Screen button a new Risk Screen row populates in the Risk Screen grid as well as an editable Risk Screen form.
Note: Previous information from the most recent Risk Screen auto-populates into the new entry.
2. Modify the following fields as appropriate:
- Recorded By: Autopopulates with PARIS AUTO GENERATED and cannot be changed (user information will be captured in Comments/Control Plan field as signature)
- Team Name: Autopopulates with PROFILE EMR and cannot be changed
- Date Recorded: Autopopulates with Today and can be changed

Visit type: Autopopulates according to the below sequence and can be changed
- First Risk Screen will always be Pre-Visit
- If Pre-Visit Risk Screen exists, next will be Initial Visit
- If Pre-Visit AND Initial Visit exist, next will be Review Visit
- If Pre-Visit AND Initial Visit AND Review Visit exist, next will be Review Visit
- Review Date: Autopopulates with Date Recorded + 1 year and can be changed
- Primary information: Mandatory field (100 character limit)
3. Complete the Risk Screen entry by ticking the relevant checkboxes and adding any comments as needed. Control Plan cannot accept any entries unless a Risk has been identified. (2000 character limit)
Important! DO NOT DELETE OTHERS' ENTRIES! Comments are entered reverse chronological (most recent at top). You MUST sign at the end of each entry. This keeps history and identifies entries over time. (Hint: Typing template code for signature is s\)
4. If you identify a Risk by ticking Risk Present, the Control Plan field is now bold, underlined, and is mandatory to complete.
Important! When a risk no long exists, you can untick Risk Present. Unticking all Risk Present checkboxes will result in the Control Plan to be deleted! The Control Plan label now appears unbolded and the field is empty. In those cases where you have ended a Risk, make an entry in the Comments field to explain why the risk is no longer relevant.
5. When you have ticked a checkbox in all rows and completed all control plans (if Risk identified), the Complete? checkbox at the bottom of the screen will auto-populate as ticked.
6. Scroll to the top of the Risk Screen and click Save or Save & Close.
Note: When a Risk Screen has been marked as Completed, it cannot be modified and the summary is read-only. When a Risk Screen is not marked as Completed, it can still be modified and the summary is editable.
7. When you tick a Risk Present checkbox and complete a Control Plan, a STAFF ALERT is automatically created in Client Alerts. When you untick all Risks, the Control Plan text disappears, and the STAFF ALERT is automatically end-dated and goes into history.
Note: When you review a client record with STAFF ALERT, you know there is at least one Risk and control plan identified. View the client's Risk Screen summary to view the risks and control plans. Do not enter notes in the alert Reason field as the Risk Screen has all the information.
Document a Risk Screen Entered in Error
A Risk Screen cannot be deleted. Instead, you can mark it as Entered in Error. The Entered in Error? checkbox is at the end of the form.
1. Tick the Entered in Error? checkbox
2. Scroll to the top of the Risk Screen and click Save or Save & Close
When a Risk Screen is marked as Entered in Error, the Risk Screen grid row will have a Y in the Error? column. This grid row is then excluded from being the default Risk Screen summary and the information does not prepopulate a new Risk Screen entry.
