ISI Typing Template and ISI Form
Background
Indigenous Health Services (IHS) is leading a project to standardize how VCH and other health authorities capture Indigenous Status Identifiers (ISI). Starting March 10 2025, the Indigenous self-identification (ISI) questions are available in Profile EMR for VCH Community sites.
ISI is a process that allows clients to inform VCH staff of their Indigenous identity. This information will be used to improve service offerings to Indigenous clients.
Key Points:
- Mandatory to Ask: VCH staff must ask all clients the ISI questions.
- Voluntary to Answer: Clients can choose whether to provide this information.
Recording ISI in Profile EMR
To ensure consistency and alignment with standardized guidelines, Profile EMR users can complete the ISI questions using one of the following methods:
1. Indigenous Self-Identification Form
The Indigenous Self Identification form is launched within an encounter note using the typing template ISI\.
Note: Information completed on this form is mapped to the Client Registration Form and vise versa.
2. Client Registration Form
The ISI questions are located at the bottom of the Client Registration Form.
