Background
The Care Team is a standardized place to document the names and contact information of providers and clinicians who provide care to a client at any given time (e.g., MRP, nurse, counsellor, social worker, pharmacy etc.).
Some care team entries will populate into different places into the medical record such as: the Yellow Bar (provider and clinician names) and the pharmacy name and fax number on a printed prescription. There is also a report you can run called "My Case Load-Active Clients" which is based on Care Team entries. For more information on running this report, click Help File: EMR Reports (Stored Queries)
View Internal and External Care Team Members
The Care Team list can be accessed three ways:
- From the Yellow Bar

- From Alter Client
Note: If Care Team is open in the Medical Record, it will become read-only in Alter Client.
Add Internal Care Team Members
For the Care Team roles that display in the Client Info Panel (yellow bar) such as MRP and Alternate Provider (AP), you may also Manage Providers from the Yellow Bar. For all other care team roles, see the steps below.
Internal care team members belong to your clinic such as Case Manager or Nurse.
- Highlight the row beside the correct team role
- Begin typing the clinician’s name in the Name column. As you type, the name will will auto-populate or a list of similar names will appear. Select the correct name from the drop down list
- You can add comments as needed in the Comments column.
- Click out of the row and the EMR will ask you to Save, select Yes
Note: The roles Usual Provider (MRP) and Alternate Provider should only be used for physicians and nurse practitioners
Add External Care Team Members
External care team members work outside of your clinic such as their Family GP/NP, Specialists, or Preferred Pharmacy. These care team members appear in the External Clinicians (External Providers and Pharmacies) list.
- Same steps as above
Add Other External Care Team Members
Other external care team members are not searchable in the system. Examples are the client's home support worker, financial aid worker, members of the client's "Circle of Care" (excluding Next of Kin), or other external programs.
- Select an Other row
- In the Name field, freetext type "other" and "Other Involved - Type in Comments" will populate
- In the Comments section, type the care team member's name and contact information
Remove Care Team Members
To remove an entry, right-click on the name and select Inactive from the dropdown menu.
View Past Care Team Members
The default view of the Care Team window is Current Care Team. To see inactive care team members, change the view filter to Current & Past Care Team (Status - Open & Closed).
