Overview
- This form allows clinical staff to document, monitor, and report signs of intoxication in a structured and standardized way at Vancouver Detox Centre POS
When to use the Form
- Any patient admitted or observed for intoxication monitoring
- Recording sobering observations over time
Launch the Form
- Begin within a New Encounter
- Use typing template: imu\ to launch the form
- Note: the form status is Parked meaning entry into the form is in progress. Each time it is opened, it will display the previously entered information until the entries are finished, and the status is changed to Complete
Form Sections Explained
1. Client Details (auto-populated)
- Name, DOB, PHN, etc., are filled in automatically.
2. Encounter Type
- Select either:
- Admission
- Non-admission
(This determines if this record is linked to an inpatient stay and the program is able to pull data from this section)
3. Triage Assessment
- Select the tick boxes for all that applies (e.g., Opioids, Alcohol, THC)
- Use the Other box to type additional information
- Use your date stamp s\ when adding information to the Comments window
4. Sobering Observation (Time Series)
Use this section to record repeated observations over time:
- Click the Add button and a new row will appear to enter observations e.g. Position, RR, BP, etc.
- Each entry includes who made the observation
- 5. Discharge Criteria and Outcome
- Indicate when the patient is safe to discharge.
- Select or describe the discharge method (e.g., self, police, family)
Reopen the Parked Form
- If you initiated the Intoxication Monitoring Form, you could go to that Encounter where the form was opened and click on its hyperlink and proceed with documentation
- If you didn't initiate the Intoxication Monitoring Form, open a New Encounter and type imu\ to open the parked form
Saving and Completing the Form
- Use the Form Status dropdown to mark the form as:
- Parked (draft/in progress)
- Completed (finalized)
- Save your changes and close out the form each time you use it. To do this, click the Save & Close button at the top of the form
- All changes are audit-trailed with time and user ID.
Printing the Form
- If you need to print the form, once it is completed, click Print to generate a readable copy
- All observations and entries will appear clearly
Tips for Success
- Use typing template imu\ to access the form quickly.
- Document observations frequently during the sobering process.
- Always mark the form as Completed when finalized.
- Ask your team lead or superuser if you’re unsure about any fields.
