Background
As a part of the OOT Re-implementation Project, OOT leadership and the Profile EMR team have been working together to understand the challenges around viewing POD caseload with average client acuity score. The Caseload Manager has been designed with this in mind.
Form Features
The OOT Caseload Manager has been developed to assist OOT leadership in managing a Shared Care Model of Care. Through the Caseload Manager form, users have the ability to do the following:
- Search Individual Caseload
- Search Shared Care Pod Caseload
- Print Caseload
- View a Client Count
- View Average Client Acuity Score
- Open up the Client in Context Medical Record
- Assign a Client to a POD
- End Date a Client from a POD
Launch the Caseload Manager
1. Log into Profile EMR using a Allied Health User Account.
2. Select OOT as your place of Service.
3. Click on the Caseload Manager Icon in the top bar.
Search Functions
Search for an individual team member's caseload
Who are the clients in the case load of a specified team member ?
To search by Individual Clinician:
- Enter the name of the team member in the Individual Clinician field
- Click on Search button.
- The search result will show all clients who have the specified team member as a Primary or Secondary Worker
Search for a Pod Caseload
Who are the clients in a specified Pod case load ?
To search by Shared Care Pod:
- Click on the dropdown menu to select a Pod
- Click on the Search button
To search by Shared Care Pod and Type:
- Click on Shared Care Pod radio button. Click on dropdown menu to select a Pod
- Then click on the dropdown menu to select a Type
- Select Null to generate a client caseload that do not have a Primary or Secondary Worker within the selected POD
- Select Primary to generate a client caseload with a Primary Worker within the selected POD
- Select Secondary to generate a client caseload with a Secondary Worker within the selected POD
- Select Null to generate a client caseload that do not have a Primary or Secondary Worker within the selected POD
Search for the clients that are not yet assigned to a Pod
Who are the clients that are not yet assigned to a Pod ?
To search for clients that are not assigned to a Pod:
- Select No Assigned Pod in the dropdown list for Shared Care POD.
- Click on Search button
Medical Record Button
To open up a client medical record from the generated list:
- Click on the line item where the client name appears to put them in focus.
- Then click on the Medical Record button.
Assign Button
To assign a client in a POD Caseload to another POD:
- Click on the line item where the client name appears to put them in focus.
- Then click on the Assign button.
A box will appear with the Client in context and the Pod they currently belong to.
- In the Shared Care Pod field, click on the dropdown and select the Pod you wish to assign the Client to.
- Then click on the Save button.
NOTE: On the backend, the Care Team Role Overdose Outreach Team POD is being end dated and a new one is going to be created with the POD selected.
- A prompt will appear informing you that the Overdose Outreach Team POD Care Team has been Added. Click Ok to proceed.
- The Caseload Manager will now refresh and display the list for the POD you assigned the Client to.
End Date Button
To End Date a Care Team Role from the generated list:
- Click on the line item where the client name appears to put them in focus.
- Then click on the End Date button.
Print Button
To print a list of the current POD Caseload, click on the Print button.
Client Count Field
The Client Count Field will display the number of unique clients that belong to the POD Caseload that has been generated.
Average Acuity Field
The Average Acuity Field is auto-calculated based on the Sum of the Acuity Score for Each Client and divided by the number of clients in the POD Caseload that is generated.
