Background and Accessing the Form
The HIV Chronic Disease Management Form is used for the chronic care management of HIV. To access the form, open a New Encounter and enter hiv\.
Form Overview
The HIV Chronic Disease Management Form has two tabs and contains the following sections:

Main Form Tab
Recent Results
The recent results table will display the last two results for each of the listed labs. Abnormal results will appear in red. The most recent results will be pulled into the client's Encounter note once the form is saved.
HIV Medication Table
This table pulls in medications from the client's Prior and Usual Scripts that are related to HIV care.
NOTE: This may not be an exhaustive list of medications, especially if the medication was ordered as an ad hoc order. Always check Pharmanet if you are not sure.
New Encounter
Free text box for clinicians to document their visit notes. Anything entered into this box will be pulled into the client's Encounter note once the form is saved.
Substance Use
This section is used to document client's substance use history. Information entered into this section will be pulled into the client's Encounter note once the form is saved.
DXA Scan and Anal Pap
Clinicians can manually enter the client's most recent DXA Scan and Anal Pap results into this section. Any future forms will display the result that was entered in the last HIV Chronic Disease Management Form. This section will also display any existing DXA Scan or Anal Pap interventions, and allow clinicians to add new ones directly from the form. If a new intervention is added, a note will be added to the Encounter note once the form is saved.
HIV History
This section allows clinicians to document and review a client's HIV history. All of the read-only fields will auto populate the corresponding result from the client's Medical Record. The non-read-only fields allow clinicians to enter relevant information for the client. This information gets pulled into future versions of the HIV Chronic Disease Management Form
Drug Class
This section allows clinicians to document any mutations to certain HIV treatments that may arise for the client. Information entered into this section will be pulled into the client's Encounter note once the form is saved.
Immunizations Tab
Immunization Type
There are currently twelve immunization types available to review in this table. Prescribers can expand each row to see more information about each immunization related to the client.
Notes
- Discussed and Declined checkboxes: Ticking the boxes will pull "Discussed" and/or "Declined" into the free text field below (depending on which was ticked).
- Free text field: Allows clinician to enter notes regarding visit. These notes will pull into the Encounter note when the HIV Chronic Disease Management Form.
- Read-only free text field: Displays notes from previous HIV Chronic Disease Management Form.
Immunization History
This table will display any completed immunizations for the client of that immunization type.
Actions
- Existing Intervention for ...: Will display an existing incomplete intervention for the client with the intervention's due date.
- Add ... Intervention: To add a new intervention, enter due date into date field and click on the "Add ... Intervention" button.
If a new intervention is added, a note will be added to the Encounter note once the form is saved.
