Background and Accessing the Form
The Type 2 Diabetes Management Form is used for the chronic care management of diabetes. To access the form, open a New Encounter and enter t2dm\.

Add Diagnostic Codes to Problems List
Select any of the checkboxes to add the associated diagnosis to the client's Problems List. If the client already has that diagnosis in their Medical Record, the checkbox will be read-only.
Subjective
Free text box for clinician to enter visit notes related to the visit subjective.
Objective
- DON'T add labs to Encounter Note checkbox: Tick box if you don't want the most recent results to populate in the Encounter note when the form is saved.
- Read-only fields: Pulls in corresponding result if it exists in the client's Medical Record.
- BP, Weight, and PR fields: Enter values to create new measures.
- Graph buttons: Click to see cumulative graphed view of results/measures.
- Free text box: For clinician to enter visit notes related to the visit's objective.
- Foot Exam & Monofilament: To document if exam was completed as well as the completion date.
- Diabetic Eye Exam: To document if exam was completed as well as the completion date.
Assessment/Plan
Free text box for clinician to enter notes related to the assessment/plan for the client. The read-only box below will display the assessment/plan from the previous T2DM form.
Current Meds
Pulls in the client's current medications. Clinician can use the Go to Scripts button to review the client's medications directly in the Scripts module.
Discontinued DM Meds
Pulls in client's discontinued DM medications. Clinician can use the Special Authority eForms button to take them to the Special Authority eForms website.
Forms and Interventions
- DM Clinic Referral Form (SPH), DM Clinic Referral Form (VGH), Inlow's 60 Basic Foot Form, Hypoglycemia Handout, CPG Interactive Decision Tool buttons: Click to open each corresponding form.

Order DM 2 bw (pre-filled): Click to add diabetes management bloodwork to the Encounter.
- Add Intervention: Click to create a new intervention for the client (will open the intervention window).
- Last Specialist Appointment: Displays information from the last T2DM form. Clinicians can manually enter the client's last specialist appointment information in this field.
