Overview
As a part of the TOP Collaborative Initiative, the SAM Tool Screener was created to identify, screen, develop and review treatment plan efficacy for new and current clients who may meet the criteria for Treatment Resistant Psychosis (TRP). This tool will support Mental Health and Substance Use (MHSU) prescribers in determining if a client is eligible for a trial of clozapine medication for the treatment of Schizophrenia or Schizoaffective disorder.
The S A M Tool Contains three sections:

Form Features
The Sam Tool Screener form has been designed to assist users with documentation. Below is a list of features built into the Form to streamline the documentation process.
- Mutual Exclusivity
- Only one of two (Sam Tool Screener or Clozapine Start) forms can be opened or active at a given time
- The Form Status
- Field options: Parked or Complete
- Sequential Flow
- Form follows a predetermined order
- Users must answer questions sequentially
- Conditional Logic
- Availability of subsequent questions depends on responses to previous ones
- Save and Resume Functionality
- Allows users to save progress
- Permits the resumption of the form at a later time
- Documentation Tracking
- Users name and time stamp are documented when a question is answered
- Mandatory fields
- The form will not save until the mandatory fields are complete. A prompt will appear to notify you of the field that needs to be completed in order to save form.
Launch the Sam Tool Screener
- Click on the New Encounter Icon from the toolbar
- In the body of the New Encounter, type in sam\
- The SAM Tool Screener Summary form will appear.
S A M Tool Screener Summary
The SAM Tool Screener Summary serves as the initial interface for users, providing a snapshot of the current status and outcomes of the screening process. Its primary functions are to display real-time screening statuses and outcomes, along with a comprehensive history of documented fields from each screening session.
Screening Status and Outcome
The Screening Status and Outcome fields are updated in real-time when the screener is saved, ensuring the display of the most recent information regarding the screening process.

Screening Status Definitions
- Screening in Progress: Indicates that the screening process is actively ongoing.
- Screening Complete: Signals the conclusion of the screening process.

Screening Outcome Definitions
- Below are examples of some of the Screening Outcome you will commonly see:
- To Be Determined: The outcome is pending further evaluation or input.
- Adequate Response Achieved: Denotes a positive response meeting predefined criteria.
- Meets Criteria for TRP (Suitable for clozapine): Indicates eligibility for clozapine treatment.
- Meets Criteria for TRP (Not Suitable for clozapine): Recognizes suitability issues for clozapine treatment.
- Does not meet the criteria for TRP (Not suitable for clozapine): Indicates ineligibility for clozapine treatment.
Form History
The tables below provides a historical record of each unique form, offering insights into the evolution of screening sessions. The table enables users to be able to review documented fields from each session, facilitating a comprehensive understanding of the screening process over time.
Sam Tool Screener Table
To access the version history for a specific screener, use the top table, known as the SAM Tool Screener table. This table displays each unique form that has been filled out. Choose the desired form from this table, and the Version History section, located in the bottom table labeled Sam Tool Screener Version History, will display all the previous entries related to that specific form.
Sam Tool Screener Version History Table
Each time a SAM Tool Screener form is saved, the system will automatically create a version history entry in the bottom table, named Sam Tool Screener Version History. Each time you save and select Continue Screening, a new version is added to this history.
Note: All version histories are read-only and cannot be edited, ensuring the integrity of the screening process.
Sam Tool Screener Summary Form Buttons
Help Button
The button located on the form will trigger a window to appear with the relevant resources for this tool. Ensure you review all appropriate resources prior to completing the screener.
The buttons located at the top right-hand corner of the form serve different functions.
View Details Button
- Description: This button allows you to view a read-only version of the selected form.
- Enabled: When there are 1 or more form entries in the table.
Create New Button
- Description: This button is used to create a new form entry.
- Enabled: When there are no form entries in the table or if the most recent entry has a Screening Status of Screening Complete.
Continue Screening Button
- Description: This button is designed for continuing an in-progress screening entry.
- Enabled: When there is an in-progress entry active in the table.
- Disabled: When the most recent screener is complete.

SAM Tool Screener Entry Form
The entry form is designed to help you document the screening details that were conducted gradually. The form is meant to be completed over time. This screener can take weeks, months or years depending on which interventions they have already tried. The minimum timeframe a screener can be completed is 12 weeks (client would have been on two medications every 6 weeks).
Begin by navigating through the form from top to bottom, answering questions as you go. Only one question is available when you start and the rest are greyed out and as you provide answers, the next question becomes available, simplifying the process. Don't worry about finishing in one go—use the "Save & Close" button at any point to save your progress. To continue or complete the screening later, click the "Continue Screening" button. This allows you to revisit your in-progress form, update answers, and repeat these steps until the screener is fully filled out. Keep an eye on the SAM Tool Screener Summary View to see your latest entries each time you save the form. Follow these steps, and you'll navigate the SAM Tool Screener effortlessly.
Launch Sam Tool Screener Entry Form
1. Click on the Create New button in the Sam Tool Screener Summary Page.
The initial segment of the Entry form is now open for completion.
Note: Intervention button are seen throughout these forms. You will notice that there are due date and frequency fields that have been set by default. When you click on the Intervention button, you will see a pop up that will confirm that the Intervention has been created and the option to go to the Intervention Set module to review and modify the Intervention if applicable. Please note that in order to see the new intervention that you created, you will need to click on the refresh button.
State of form for net new form:
- Initial Screening Date: Todays date will auto populate. This date is the date the screener was first created.
- Current Screening Date: Todays date will auto populate. This date reflects the current screening date.

POS: The Place of Service (POS) will auto populate to the POS you are currently logged into.
- Note: This field by default read only. Please ensure that you are logged into the POS where the Client is receiving service.
- Screened By: Will auto populate logged in clinicians name

Intervention Button: If an Intervention is required to support completion of the Screener, by default the intervention is set to be due at 8 weeks intervals.
- Note: The Due Date and Repeat Intervals can be manually changes as applicable.
Fill out the rest of the form and based on the answers you select, the form will either enable or disable fields to support the screening process.
Note: The screener will provide guidance advice based on your selection(s).
Diagnosis
You will only be allowed to progress through the screener if the Client has Schizophrenia or Schizoaffective Disorder.
If the client has Psychosis NOS or No Other Relevant Diagnosis, guidance advice will appear that the form does not need to be filled out at this time.
Clozapine Status
If the client is Already on Clozapine, select the Yes radio button to complete the Screening. A guidance note will appear with a Clozapine Start Form button. Click on the button to begin documenting the dosage and optimization of the client’s current clozapine trial.
Trialed Antipsychotic Medications
There is two trial antipsychotic medications data entry fields. The client may have tried more than two but only the two most recent antipsychotic medications should be recorded here. You can choose to include a depot medication as one of the two medication, however you do not need to. You can list all other medications trialed, along with their outcomes in the medication outcome information field below.

Treatment History
Selecting the Yes or No radio button will allow you to move forward with screening. If no is selected, guidance directions will appear to consider adherence aids as a clinical queue, otherwise provide reasons for non-adherence in the reason for non-adherence box.

Psychoeducation
In order to complete screening you must provide formal psychoeducation to the client regarding the client’s mental health condition.
NOTE: If No is ticked, you must first provide education then come back to the screener to update the answer to Yes. On selecting Yes, you will need to provide a date that the education was provided in order to move forward. This date is a mandatory field for reporting purposes.

Depot
If you have not offered a depot, there is direction provided for you to consider to offer/start a depot, if appropriate, and return to screener to indicate response to depot if depot has been started.
- If you have not offered your client a depot, consider offering a depot in order to determine if your client meets the criteria for treatment resistant psychosis
- If you are not offering your client a depot or if you have offered but the client has not accepted, provide a reason in the additional information section below.
Treatment Resistant Psychosis Outcome Summary
Client's who are eligible for Treatment Resistant Psychosis (TRP) must meet the one of the following criteria:
A) Tried two anti-psychotic medications and have been adherent to treatment without an adequate response
or
B) Tried two antipsychotic medications, have been non-adherent but have been offered or trialed a depot medication (if appropriate) without an adequate response.
- If No is selected to either of the questions in this section you will need to provide a reason as to why they did not meet criteria for TRP or why they are not suitable for clozapine in the Additional Information box
- If the Client does meet the Criteria and is Suitable for Clozapine, you can continue to the Clozapine Start Form.
