Background
This form is for the use of clinicians who are conducting Fall Risk Assessments on all or some of their clients. Currently this is a requirement for certain acute care facilities, for each client over the age of 65 as well as any younger client whom they perceive to be at risk for falls.
Using the Form
Access the form from a New Encounter
Fill out the form using these steps:
Open a new form on each visit. Previously entered values are saved in the form and can be edited. The form will show the Initial Intake Status as well as the three last visits. Forms saved on previous visits can be viewed in the client’s Medical Record, Forms. The default view is for Forms Created within 90 days. Click on the drop-down arrow to find older forms.
