Background
The Unsigned Results Report contains certain results and reports that have not been signed off at your clinic for 7 days or more. Originally the report showed only the unsigned Pathology and Scanned Documents categories.
Now, it will show more categories and include: Admission and Discharge, Advance Directives, Cardio Respiratory, Complaints, Rx Refills, Appt Info, Consents, Information Disclosure, Information Request, Consults, Hosp 3rd Party, Transcriptions, Pathology, Radiology.
It is recommended to review and have Unsigned Transactions signed-off on a weekly basis.
Note: The Unsigned Results report may also be referred to as the Unsigned Transactions Report.
Access the Unsigned Results Report
The Unsigned Results report is on an excel spreadsheet and located in a secure website outside of the EMR. The username and password to access the secure website can be found in the Unsigned Results/Transactions Instruction document in the EMR Library. The password to access the report is changed every two months.
1. From your Work Centre, click on the Other tab, and click on the Library icon.
2. In the Look for: field, search for Unsigned.
3. Double-click on “UnsignedResults” to open the Unsigned Results/Transactions Instructions document.
4. Follow the instructions in the Unsigned Results/Transaction Instructions document to access the Unsigned Results report outside of the EMR.
Note: There is a specific username and password you must use to access the Unsigned Results report which is contained the Unsigned Results/Transaction Instructions document.
Understand the Components of the Unsigned Results Report
The lab sends the results to the clinic that ordered the test. In this report, this is known as the Transaction Clinic (POS). The Client’s Clinic is the clinic that displays in the client’s yellow banner.
Note: Each POS has its own lab identifier known as the C number. If a clinician is a member of multiple clinics, it is imperative that they log into the correct POS to order the lab test so that it goes to the correct POS.
Filter the Report to Find Your Clinic/Clinician’s Unsigned Results
To filter by the Transaction Clinic:
1. Click on the Sort & Filter icon.
2. Click on Filter from the dropdown.
3. Click on dropdown arrow and select a filter as applicable. For example, if you only want to see results for your clinic, untick the (Select All) checkbox from the filter and then check off your clinic’s code.
Get Unsigned Results Signed Off
All clinics are responsible for getting their clinic’s unsigned results signed by the appropriate clinician. If your clinic (or one of your clinic’s staff) is listed in any field on the Unsigned Results report, do the following:
1. Once you’ve filtered the unsigned results for your clinic, use the PARIS IDs to open the respective client’s Medical Record.
2. Determine which POS the client belongs to or most frequently attends, by either:
i. Looking at the yellow banner for a Primary Care clinic (PC) and/or a
Most Responsible Provider (MRP), or
ii. Clicking on the Bookings icon and looking for a physician/NP they have seen recently.
3. From the unsigned result in the Medical Record, create a task to the Physician/NP group of the respective POS.
i. From Results in the Medical Record, select the unsigned result and click on Annotate.
ii. In the Annotate window, type “Unsigned Result over 7 days”.
iii. Assign a Task to the Physician/NP group of the POS that the client belongs to. This way, if the ordering provider is not available, the Clinician of the Day will pick up the unsigned result.
iv. Click Annotate.

What to do if the Transaction Clinic, Client’s Clinic, and Assigned To fields are NULL in the Report
For Transaction Type = Pathology:
If the Transaction Clinic or Client’s Clinic and the Assigned To fields are NULL, the result has not been assigned to a clinic or a clinician. It is most likely sitting in Manual Matches in the EMR and must be matched before it can be signed off.
If the client belongs to your clinic: in Manual Matches, match the result to the correct client or clinician. Once matched, the result will move to the clinic’s Unsigned Transactions list and the client’s Medical Record. This is where the clinician will sign off on the transaction.
If the client does not belong to your clinic: in Manual Matches, print the transaction, then fax and notify Excelleris that the client does not belong to your clinic. Delete the result from the Manual Match list.

For Transaction Type = Scans:
If the PARIS ID is listed but Assigned To is NULL, then the document has not been attached to a clinician and cannot be seen in the clinic’s Unsigned Transactions list in the EMR.
To move the document into the Unsigned Transactions list:
1. Go to the client’s Medical Record. In Results, right-click on the scanned document and select
Set Properties.
2. Correctly assign the document to the provider and POS; now the transaction will move to the clinic’s Unsigned Transactions list.
