Background

EMR refers to both results and transcriptions as transactions. In order to receive electronic results/transcriptions from Excelleris, the clinician needs to be set up correctly in the lab, transcription, and Excelleris' systems. The clinician needs to fill out a form and send it to all three aforementioned parties.

Excelleris Acceptable Use Authorization (AUA) Form

Excelleris requires an AUA form before it will send electronic results to the clinician. 

• The form should be completed once by the clinician.  If the clinician is working at another VCH Profile EMR clinic, and is actively receiving results electronically, this form is not required.  Excelleris just needs notification that a clinician is working at another clinic.
• It should be signed by the clinician, and then sent to Excelleris via email or via FAX as per the information at the bottom of the form. See screenshot for further instructions.

To access this form:

1. Log into Profile EMR
2. Select icon on the Far right tool bar "Admin Forms"
3. Select the item "Excelleris AUA Form"
   
4. Complete the form with clinician information including MSP #(see screenshot below) 
5. Save as a PDF
6. Clinician should sign the form in PDF format if possible, or print it and sign the form
7. Send the form via email or fax to the instructions at the bottom of the form.

Provider Information Form (info to Labs and Excelleris)

This is a form that a CSC or Clinician could access within Profile EMR. 

The form can be used to request:

1. New distribution (e.g., provider new to EMR or new to POS)
2. Change distribution (e.g., POS name or address change)
3. Remove distribution (e.g., departing provider)

To access this form:

1. Log into Profile EMR
2. Select icon on the Far right tool bar "Admin Forms"
3. Select the item "Provider Information Form (info to Labs and Excelleris"
   
4. Complete the form with clinician information and verify POS information
5. Print as a PDF
6. The form must be sent to all of the lab, transcription, and Excelleris systems (see instructions for sending at the bottom of the form)
