What was the issue?
- On April 27th, our team published a Safety Advisory which highlighted that a subset of toolbar icons were not behaving as expected.

Our team has completed a fulsome investigation and has confirmed this issue would occur:
- If a client (Client A) was placed into context in the Yellow Bar via an Active Form (e.g. PWD Application, Caseload Manager, Discharge Form, etc.), AND
- You had a medical record for a different client (Client B) open in the background, THEN
- Clicked on an affected Toolbar Icon - This resulted in the icon actioning/opening for the client whose medical record was open in the background (Client B), NOT the client who was last in context in the Yellow Bar (Client A)
What has been fixed?
- Client Registration
- Risk Screen
- Medinet
- PARIS Quickview
- CareConnect
- Pixalere
- PARIS Update
What has NOT been fixed?
- The New Task and Phone Call icons have NOT been fixed!
- The vendor is aware of this issue and investigating a solution for same.
Recommendations for Using the New Task and Phone Call icons:
- Ensure the medical record of the client is open/active
- Always double-check the task/phone call has been assigned to the correct client proceeding.

April 27, 2026: Subset of Grey Tool Bar Icons are Not Actioning as Intended
Summary of Issue
A number of our Grey Tool Icons are not actioning/opening as expected.
As a result of same, users may see Tool Bar icons actioning for the wrong client (e.g. New Task created for the wrong client, Client Registration form opens for the wrong client, etc.)
How is this happening?
Our team has confirmed that this issue will occur under the following circumstances:
| User opens medical record for Client A - client A is 'in context' in the Yellow Bar | |

User selects Client B, Client A is replaced in Yellow Bar by Client B *This can be done via Appointments, Case Load Manager Tool, Stored Query, etc.) |

User clicks on Affected Grey Icon via Grey Tool Bar *Affected Icons currently include:
|
|
| Clicks on the new task icon in grey tool bar → new task window opens and → regardless of whether user selected Save or Don’t Link, the following occurs |

Observed Results:
- Even if Client B was 'in context' in the Yellow Bar, when a user clicks an affected Toolbar Icon, the subsequent tool will action/open for Client A
- The user will notice that the Yellow Bar reverts back to Client A
- This behavior occurs regardless of whether Save or Don’t Link is selected.
The below image displays this issue where Client B was 'in context' in the Yellow Bar and the 'New Task' icon was selected.
Outcome and Status
New Task Icon:
- Our vendor is aware of this issue - however there is currently no way for our team to fix this issue specific to the New Task icon
- Updates will be communicated once a vendor fix is available for this specific icon.
All other affected icons (Client Registration, Community Risk Screen Tool, Care Connect, Medinet, PIXALERE, PARIS Quick Screen):
- Our internal Profile EMR team will be able to fix the issue for the above icons and are currently working on same
Recommendations for Users
Users should be aware of this behavior and exercise caution when using any of the impacted Tool Bar icons and are strongly encouraged to:
- Work within 1 client record at a time - and to avoid having multiple client records open at a time.
- Double check the client listed in the window/form when using any of the affected Tool Bar icons.
